Investigation Summary

Employee #1 was part of a finger joint crew at a cut stock facility. He was operating the #4 finger joint machine when a block of wood became jammed. Employee #1 turned to go to the point of the jam and, as he stepped toward it, his foot became caught in the loop of twine on the 30 gallon plastic garbage can beside his work station. He lost his footing and fell off the 5-in. high work platform. Employee #1 landed on the concrete floor, fracturing his hip. The wheeled can was for waste and defective blocks of wood, and the loop was tied to the can to make it easier to move when it was full.
