Investigation Summary
Employee #1, age 19, of DuoPane Industries, a small manufacturer of double-pane window glass, was helping a forklift operator move a 4,000 lb glass crate from the west to the north side of a production shop. As he was guiding the crate, Employee #1 fell in front of the forklift, which struck and lacerated his left leg.
