Employee injured when struck by falling personnel lift

At approximately 1:15 p.m. on December 23, 1996, Employee #1, an inmate at Deuel Vocational Institution in California, was working with a supervisor to change an overhead light using a personnel lift. The supervisor had gone approximately 20 ft away to make a phone call to shut off the electricity. Employee #1 was hammering out a locking pin on the support arm that kept the personnel lift horizontal during transit when the lift fell on him. He sustained a broken leg and hip. Employee #1 should have raised the lift and taken the load off the pin instead of hammering it.
