Employee injured when loader's brakes fail

On August 26, 1996, Employee #1 was operating a Clark 75C loader at a housing tract project. The brakes failed and he lost control of the loader, which went down a slope and overturned at the bottom. Employee #1 was partially ejected from the cab and suffered a serious injury to his right arm. He was not wearing a seat belt.
