Investigation Summary
At approximately 11:00 p.m. on October 1996, Employee #1 responded to an alarm at the sand silo area where a sand chute had become blocked. He attempted to clear the jammed wet sand by reaching into the chute area. In doing so, he leaned on the scale railing. Given a false weight reading, the gate closed, catching his right hand between the chute plate and the sand within the chute. His hand was crushed and Employee #1 was hospitalized.
