Employee's ankle broken in fall from ladder

At approximately 9:00 a.m. on July 31, 1996, Employee #1, of Tri Valley Drywall, was working as a drywall hanger installing/hanging Sheetrock. Employee #1 could not find the extension ladder normally at the job site, so he decided to use his own 8 ft job-made ladder. He set up the ladder by leaning it against the frame of the structure. Employee #1 was standing on the top step handing a piece of wallboard to his coworker, who was installing pre-rock at the gable/attic area. The ladder slipped and Employee #1 fell, catching his leg on a ladder rung and falling approximately 6 ft to a concrete floor. Employee #1 sustained a broken ankle and was hospitalized.
