Employee injured in fall when wooden access board breaks

Employee #1was using a wooden board to access a location to replace a bearing on a chute gate. The board broke and he fell approximately 12 feet onto a catwalk. Employee #1 suffered only bruises and was released from the hospital on the same day. Use of the wooden board for this purpose was contrary to Employee #1's training and instruction.
