Event: 01/13/1998
Employee injured in fall from aerial lift

Employee #1 was installing a 20 ft tall by 13 ft 7 in. wide overhead door on a new building. He was on an aerial ladder, positioning the door track, when a coworker operating the lifting boom accidentally struck the lever for the ladder that Employee #1 was using. He fell somewhere between 15 and 17 ft to the ground, fracturing his hip, elbow, and several ribs. The boom operator was watching the boom instead of the controls, all of which were properly marked.
