Investigation Summary
Employee #1 had been assigned to clear debris from the water filter valve at a new water treatment plant. Every time the small filter became clogged, she was to shut off the valve, unscrew its cover, clear out the debris, reinstall the lid, and re-open the valve. The valve was becoming clogged every 15 to 20 minutes and she performed the entire cleaning routine several times under the guidance of the first-shift worker. When the filter next needed cleaning, Employee #1 took off the cover before the pressure was fully released. The cover broke loose and struck Employee #1 in the face. She suffered a fractured jaw, facial laceration, and the loss of two or three teeth.
