Investigation Summary
At approximately 8:30 a.m. on August 13, 1997, Employee #1, a lead installer/driver for Special Events, was assigned to set up a party in Milpitas, CA. After leaving the warehouse with his crew, he remembered that he had forgotten to bring some anchor bases for the ten, so he returned to the warehouse and backed up his truck onto the loading/unloading ramp. As soon as Employee #1 had stopped the truck and got out to tell the warehouse supervisor why he had returned, a coworker drove up on a forklift with an elevated pallet full of bases. The coworker left the loaded pallet about as high as the back end of the truck to facilitate their transfer. As Employee #1 and the supervisor unloaded the bases into the back of the truck, the coworker stayed on the forklift. After the transfer was completed, Employee #1 jumped onto the back of his truck, with one foot inside and the other on the bumper so he could close the back door. At this moment, the forklift moved forward and Employee #1's right leg became pinned between the pallet on the forks and the back of the truck. He sustained a fractured fibula and tibia, and was admitted to Washington Hospital for treatment. Employee #1 had been employed by the company for about 1 1/2 years prior to the accident.
