Investigation Summary

At 10:15 a.m. on January 16, 1998, Employee #1 was operating a shower mirror assembly machine. The machine's function was to force fit the back plate of the Showertek, shower mirror, into its round frame. The job consisted of placing the two parts in the machine, pushing the two control buttons, one on each left and right side of the machine, and removing the Showertek when it is finished. The sequence of machine operation is that after the two control buttons, one with each hand, are pushed, the machine's safety guard comes down to enclose the moving parts and the point of operation of the machine and then the press plate comes down to force fit the back plate on to the Showertek round frame. At the end of this operation, when the press plate begins its return stroke, the safety guard opens at the same time and the finished Showertek is taken out of the machine. After the force fit operation, when the safety guard went up and opened the machine door, Employee #1 put his left hand in the machine to take out the finished Showertek, but his hand got caught between the top of the press plate and the machine itself and was crushed. Employee #1 was taken to Columbia San Leandro Hospital where he was diagnosed to have sustained multiple hand fractures and hospitalized.
