Employee injured when forklift mast crushes foot

Employee #1 was performing minor maintenance on an forklift. He placed his left foot on the mast of the forklift to place hydraulic lines into the proper alignment just as a coworker, who was sitting at the controls, was lowering the mast. Portions of Employee #1's left foot, including four of his toes, were amputated.
