Investigation Summary
On December 12, 1997, Employee #1, a contract laborer supervised by a secondary employer at a warehouse construction site, was with two coworkers on the pallet-rack assembly on the third deck. He turned a corner and walked into a floor opening, catching himself on his right shoulder. Employee #1 sustained four fractured ribs, a dislocated shoulder, and head and neck injuries, for which he was hospitalized. The secondary employer failed to provide a guard or cover for the floor opening, and was cited for a serious violation of T8CCR 1632(H).
