Investigation Summary
At approximately 1:00 p.m. on September 11, 2001, Employee #1, a special-service Home Depot employee, was pulling down boxes of shelvings from a 12-foot-high rack. He fell 5 feet from the work platform of a rolling ladder to a concrete floor. He was transported to Eden Medical Center, where he underwent further examinations on his brain. He died 2 days later.
