Investigation Summary
At 8:15 a.m. on January 23, 2001, Employee #1 fell while walking across a 14-foot span, truss joist I-beam set on its side between a second floor mezzanine and the exterior framed wall of a residence under construction. The primary cause of accident was Employee #1 walking on a ramp which was not fully secured. Employee #1 fell from a height of 13 feet. Employee #1, an experience carpenter at the time of the accident, was on his fourth day of employment with this employer. Employee #1 as a result of his fall sustained contusions, multiple abrasions, and bruises. Employee #1 was hospitalized on the date of his injury and kept overnight for neurological observation, due to the concern of possible seizures.
