Employee Falls from Height when Ladder Structually Collapses

At approximately 2.30 p.m. on December 20, 2000, an employee was climbing a 24 foot extension ladder when the ladder structurally failed. The employee fell and as a result was hospitalized for two days with skeletal injuries.
