Employee's ankle fractured between rack and forklift

At approximately 12:10 p.m. on May 10, 2000, Employee #1, a warehouse laborer for Select Personnel Services, was assigned to work at Pacific Cheese in Hayward, CA. His left ankle became caught between a forklift and a storage rack, and was fractured. He was transported to Saint Rose Hospital, where he was treated and released the following day.
