Investigation Summary
On October 19, 2000, an employee, an order selector, was operating an electric pallet jack in the warehouse. While moving the pallet jack in reverse he tried to avoid backing into a storage rack. In the process the employee placed his right foot down in the path of the pallet jack which caused his leg to be pinned between the pallet jack and a stack of pallets, breaking his right foot and ankle. The employee was hospitalized and treated for fractured foot and ankle.
