Employee injured while operating a forklift

On April 27, 2002, an employee working as an EVS worker was employed at a medical treatment facility, Kaiser Permanente, in Oakland, California. At the time of the incident, the employee was not certified or trained by the employer to operate the Clark Lift-industrial forklift (SN 3274LG). The employee was engaged in moving some cardboard bales from a loading dock area to a recycling cage when he sustained a fractured femur, right arm, and ankle.
