Employee Suffers Fingertip Amputation

At approximately 11:30 a.m. on June 2, 1999, Employee #1 was servicing a drive-motor that was located about 15 ft above the floor and positioned under the overhead conveyor system. Employee #1 turned off the machine, serviced the unit and restarted the motor with the chain guard removed. Employee #1 noticed something on the moving sprocket. Wrapping a rag around his finger he wiped the sprocket while it was running. The drive unit caught the rag and pulled Employee #1's left index finger into the point of operation. Employee #1 suffered a left index finger tip amputation and was hospitalized.
