Employee's finger tip amputated in roller machine

Employee #1 was rolling black sheet metal into a pipe roller. He was standing in front of the roller with his foot on the feed and direction control button and his right hand on the pipe being rolled. He hit the wrong direction button and his right hand and finger tips were pulled into the point of operation. He crushed and amputated the tip of his middle finger.
