Investigation Summary
Employee #1 worked at a city water treatment plant in Lakeport, California. He was standing next to an air exhaust fan for an ozone treatment station. The 10-in.-Cincinnati fan was enclosed in an aluminum housing with a 4-in. exit port, and lacked a guard. Employee #1 put his right hand in front of the fan's 4-in. opening in an attempt to muffle the fan's noise. He inadvertently let his fingers enter the exhaust port of the fan housing, and lost his two middle fingers to the first joint.
