Employee Killed Struck By Large Pipe

Employee #1 worked for Team One Personnel Solutions, a contractor for and supervised by Napa Pipe Corporation. He was sweeping chips from the floor below the rails. The pipe rolled between the "A" face mill and the "A" inspection table. He faced away from the incoming pipe, approximately half way between the north rail and the end of the pipes. He then faced the stationary pipe on the rails awaiting transfer for inspection. He entered the area of pipe flow and turned his back to the area from which the pipe normally flowed. The pipe crushed his head and chest, killing him. The steel pipe measured thirty inches in diameter. The thickness was approximately 0.5 inches. The length was approximately forty feet. The pipe was fifty six inches above the floor.
