Employee falls from roof

Employee #1 was walking on sloped plywood roof sheathing, carrying a bundle of shingles. Employee #1 slipped, slid down roof, held on briefly at edge, then fell 17.5 ft from the eaves to the lawn. He fractured the wing bones on two vertebrae. Employee #1 inspected the roof for dampness and debris which could cause slipping prior to starting loading but must have missed a wet spot.
