Investigation Summary

At approximately 2:00 p.m., on February 7, 2001, Employee #1, Safety Engineer, for Sir Francis Drake Hotel, suffered a serious injury to his tailbone and elbows while he was walking down the stairs, Employee #1-#3 were walking down a flight of stairs, from the 21st floor to the 20th. About half way down the stairs, Employee #1 slipped and fell back and bounced three stairs down on his rear. There was a man sweeping the stairs with a damp mop. However, the stairs were not slippery or wet. All stairs have slip-resistant material to prevent it from being slippery. Employee #1 bruised his tailbone, forearm, and elbows. He was moved to the St. Francis Emergency Room. Employee #1 was hospitalized.
