Investigation Summary

Investigation Nr: 201183373
Event: 03/25/2007
Employee Lacerates Hand While Using Dough Roller

On March 25, 2007, Employee #1 was working as a Roller at a Pizza Parlor. The dough roller or dough brake machine (EMW, Serial Number 2077086) was equipped with a stop-bar which, when actuated, would automatically shut off the power and apply a brake to the rolls. The dough is mixed in a dough mixer (Hobart) and then made into crusts in the back room on a dough brake machine. At approximately 11:30 a.m., Employee #1 was standing on an inverted plastic crate in front of the dough brake machine. The floor is tiled and slippery. According to coworkers, the crate slipped and Employee #1's left hand slid under the stop bar of the dough brake machine causing laceration on three of his fingers before the machine was (automatically) stopped. Employee #1 suffered only minor injury (laceration) and was not hospitalized.
