Investigation Summary
At approximately 11:00 a.m. on September 17, 2002, Employee #1 placed a 22-ft-long extension ladder on top of a dog house that was 3 ft to 4 ft above the ground to reach the exterior wall at the rear of the house. His coworker, who was the son of the company owner, asked Employee #1 to bring an 8 ft stepladder up to him. Employee #1 picked up the stepladder and started climbing up with it. The extension ladder began to wobble under his weight. Employee #1 lost his balance and then jumped off to avoid an uncontrolled fall. His right side struck against the side of the doghouse, fracturing four of his ribs. Several factors contributed to this accident. The doghouse was an inappropriate surface on which to mount the extension ladder, which was also not secured in any way to the surface. In addition, Employee #1 should not have been carrying a stepladder as he ascended because it prevented him from using both hands to hold onto the extension ladder.
