Employee Injured When Struck By Falling Object

At approximately 11:00 a.m. on April 3, 2002, Employee #1, a steel worker (welder foreman), was at the west side of the building setting slider plates to pick them off the truck and then fly them over the fence and put them on a stack of plates. Each slider plate was approximately 10-ft-7in. by 10-ft-2in.. Employee #1 (a certified welder) welded a 5-in.-by-6-in.-by-0.375-in. plate with a hole on to the slider plate to use as a picking point or picking eye. As they were attempting to set their third slider plates, one of the two picking eyes failed at the welded point and broke loose. Employee #1 had just completed welding the clip and was walking toward the landing area. The plate edge went into the ground and the plate fell away from the crane and hit Employee #1 in the shoulder, knocking him over and fractured his wrist and pelvis. Employee #1 was hospitalized.
