Employee injured by fall from extension ladder

At 1:55 p.m. on May 3, 2001, Employee #1, who had been working as a painter for 21st Century Home Improvement for 2.5 months, suffered a serious injury to his right leg. Employee #1 was assigned by his foreman to sand a window on the north side of the building. He placed a 28-foot metal extension ladder between the wall and the neighbor's light well on the roof. Then he placed a piece of plywood under the legs of the extension ladder on the tar and gravel of the roof. The pitch or angle of the extension ladder was approximately between 55 and 65 degrees. The ladder legs slipped and Employee #1 fell approximately 15 to 19 feet and landed on the concrete around the neighbor's light well. Employee #1 seriously injured his right leg and was hospitalized with a fracture.
