Investigation Summary
Employee #1, age 19, was engaged in forest restoration work on private land that had been burned in a wildland fire. He was cutting and chipping dead trees, and then spreading the chips over the several acre site. Employee #1 was in a Caterpillar 518 grapple skidder, serial #119-26464, backing down a 22.1 degree ravine to retrieve some logs when he began going to fast and lost control of the vehicle. He was ejected as the skidder rolled over several times down the steep incline. Employee #1 was killed. He was not wearing his seat belt.
