Employee's fingers smashed in punch press

Employee #1, age 20, was using a Bliss 30-ton punch press, serial #H19336, to punch out 2 in. by 14 in. blanks from 14 1/2 in. by 48 in. stock. There was an expanded metal barrier guard in place, but Employee #1 reached around the unguarded sides and pressed the foot switch. The press cycled, smashing two fingers on his right hand.
