Employee injured when struck by steel disc

Employee #1 was working in a production welding shop with an 84 in. diameter stainless steel disc. He was using two disc clamps, each outfitted with three bolts to secure them to the disc, to flip it over. One clamp was secured with only one of the three bolts on one side and it came loose when Employee #1 was flipping the disc. He was struck in the face by the 310 lb disc, and then became pinned under it. Employee #1 was hospitalized for treatment of facial fractures and a fractured ankle.
