Investigation Summary
Employee #1 was trying to climb from a floating work platform onto a fixed concrete dock with a welded metal extension platform located at a ferry terminal. He was supporting himself with his arms on the extension platform but could not pull himself the rest of the way up. He was lowering himself back to the float when he lost his footing and fell approximately 5 ft onto the floating platform. He landed on his tailbone and sustained bruising to his lower back. There was no access ladder to the dock or extension platform at this location.
