Investigation Summary
At approximately 2:30 p.m. on January 7, 1999, Employee #1 was replacing nitrogen gas tanks while a coworker, his foreman, was cleaning/washing the bottling line. At about 2:45 p.m., the foreman instructed Employee #1 to use the hot water pressure washer to clean the bottling tank. Just before 3:00 p.m., the foreman decided to check on Employee #1's progress because he hadn't seen him for a period. The foreman used a portable ladder to access the top of the tank and found Employee #1 unconscious inside. The foreman ran back to office and instructed coworkers to call 911. Another coworker then jumped into the tank and stayed between 1 and 2 minutes, administering CPR to Employee #1. The foreman and a coworker then grabbed Employee #1's legs, pulled him up and out through a manhole, and lay him on a picnic table. The foreman then performed mouth-to-mouth resuscitation on Employee #1 until the paramedics arrived, but he died.
