Employee's eyes burned when splashed by chemical

On September 24, 1998, Employee #1, a lab assistant, was rotating specimen containers in the morgue room, which was approximately 100 ft from the pathology lab. She grabbed a container by the top, but the lid was not on tight and the container fell out of her hand. The bottom dropped on the floor and the specimen solution, which was 10 percent formalin, splashed up into her face and eyes. Employee #1 washed her face and eyes in a nearby sink. She went back to her lab, with her eyes in pain, and was rushed to the Occupational Health office, from which she was immediately sent to an ophthalmologist. Employee #1 was then referred to the emergency room and hospitalized for 27 hours. She was not wearing gloves, goggles, or a face shield at the time of the accident.
