Investigation Summary
Employee #1 was sitting on a metal stool, adjusting the circuit board panel of a mechanical power press. His right hand was in its point of operation when he slipped off the metal stool. His left hand depressed the left run-inch button, and his head jerked forward, depressing the right run-inch button. With the two-hand controls activated, the top ram came down, crushing Employee #1's right hand between the top and bottom die. The employer was cited for a violation of T8CCR 3202(a), for failing to have a written Injury and Illness Prevention Program (IIPP); for a violation of T8CCR 3314(f), for failing to have a written lockout/tagout program; for a violation of T8CCR 4202(c), for failing to maintain inspection records for the press; for a violation of T8CCR 342(a), for not reporting the accident to OSHA; and for a serious violation of T8CCR 4193(e)(1), for failing to protect the two-hand controls against unintended operation.
