Employee crushed by unstable crate

At approximately 9:40 a.m., Employee #1 was in the middle of uncrating and relocating large sheets in separate storage stalls, and sweeping up debris. The last crate to be unloaded, weighed between 1,300 to 1,500 lbs and was precariously balanced on its horizontal edge and not blocked or restrained to prevent it from falling over. The crate fell over onto Employee #1 pinning him to the wall and floor. He was hospitalized with two broken arms, a broken right ankle, and multiple glass cuts to his body.
