EMPLOYEE DIES WHEN MACHINE FALLS ON HIS CHEST

Employee #1 was working just inside of a small maintenance building where he was changing scraper blades on a mechanical 4-bed capacity planter. He elevated the planter using a tractor on wooden blocks. He then crawled underneath it on his back to address the repairs. The planter was inadequately blocked and cribbed. In fact, all of the blocks were located at the rear of the machine. The balance of the machine was extremely precarious with a strong potential for the machine to inadvertently shift and fall forward. He apparently shifted his position underneath the machine. His new position changed the balance of the machine. The machine ultimately fell forward on to his chest. He suffered massive injuries to his chest. He died within hours at a local area hospital. He apparently worked alone. No witnesses observed the incident.
