Investigation Summary
Employee #1 was loading material onto a mezzanine level using a wood pallet attached to forks of a forklift. He was holding a cubicle board while being elevated when the material shifted causing him to fall approximately 10 feet to the ground level. The wood pallet platform did not have guardrails and was not secured to forks or a mast. Employee was hospitalized with fractured bones.
