Investigation Summary
On June 24, 1998, Employee #1 was the machine attendant on the wrapping machine in the packaging department of a chicken plant. The line consisted of long trays of chicken parts being wrapped with stretch wrap film. Employee #1 attempted to remove some stretch wrap film that had become caught in the gripper chain area. Employee #1 did not shut down the machine prior to pulling/repositioning the film wrap as required by company policy. As a result, Employee #1 lost the fingertip of the middle finger of his left hand. No hospitalization was required at the time of the incident.
