Employee's arm crushed in paper processing rollers

Employee #1, the third hand at a paper processing area, was at the dry end, where the paper break occurs. When the paper break occurs, all workers are supposed to sound a horn twice and then proceed to their positions. The backtender, the fifth hand, and Employee #1 all responded. Once the paper cut was made, the fifth hand passed the tail end of the gloss calender over to Employee #1, who should have hand fed it into a rope system attached to the Autoflyte roller. The rope drive was not on its pulleys, however, so Employee #1 reached beneath the scanner to hand feed the stock directly into the rollers. His right arm became caught between the cylinder and rollers and was pulled in and crushed. Employee #1 should have waited for the machine to be shut down, and for the rope assembly to be put on pulleys, rather than trying to hand feed the stock himself. The employer had written a safety program specifically for third hands, which detailed these procedures. The employer was cited for a violation of T8CCR 4070(f), for failing to guard the rope and pulley.
