Investigation Summary
At approximately 1:45 p.m. on March 24, 2004, an employee, working as an apprentice roofer, was attaching 4-ft by 8-ft water proofing membrane insulation, to the metal roof. The employee was moving backward using a Dewalt screw gun attaching screws to the insulation. A witness stated that the employee moved underneath the twine warning tape which was set up 13 ft from the edge of the roof. The employee stepped backward off the flat roof and fell 19 feet to the pavement. The employee was hospitalized and treated for head injuries.
