Investigation Summary
Employee #1's fingers were severely crushed when he accidentally placed his hand inside the frame of a coffee grinding machine. The tray shake eccentric was accessible through the side of the machine. Employee #1 was working nearby and slipped on coffee dust and reached his hand out. His hand was able to go through the frame rails and enter the drive system. As a result, two of his fingers were crushed and one was amputated near the tip.
