Investigation Summary
At 10:30 a.m. on July 11, 2001, Employee #1 was going about his assigned task unloading an irrigation pipe from a rear trailer attached to a fully functioning driverless tractor. This tractor did not have any other external controls for the brake, throttle, or steering installed. Employee #1 was standing in a crossways direction to a furrow. As he lifted the pipe, his foot slipped off of the edge, and he slid down into the furrow and directly under the wheels of the moving rear trailer. The trailer ran over his leg, and Employee #1 sustained a broken right ankle and was hospitalized.
