Employee Falls Through Roof Opening While Cleaning Up Trash

On July 8, 1998, Employee #1 and two other employees were given a job assignment to clean-up debris, sheetrock, paper, plywood, and miscellaneous trash on a roof. Additionally, they were asked to remove the items and place them into a debris box that was mounted on the forks of a forklift that was positioned on the roof. Employee #1 was last seen on the roof near the debris box. He was later found by a foreman on the ground beneath the roof opening complaining of injuries. Apparently, Employee #1 fell through a roof opening. Investigators were able to determine that he fell through an opening in the roof by locating a cover that was placed over the roof opening, the location on the roof where he was last seen, and the injuries sustained. No witnesses observed the incident.
