Investigation Summary
At approximately 2:00 p.m. on June 26, 2008, Employee #1, a carpenter foreman, fell 30 ft as the elevated form traveler on a 245 ft tall highway bridge, rolled off of the end of the supporting rails. He had earlier directed his crew to remove the safety stops on the rails that prevent over travel of the form. He was hospitalized for several lumbar fractures as well as a fractured right ankle.
