Employee's Finger Is Amputated When Crushed Under Load

On July 12, 2008, Employee #1 was working at a winery. He was in the process of assisting the lift of a 10-in. wide by 12 ft long, 380 pound forklift extension. Employee #1 and a coworker were on opposite ends of the extension which was being supported by forks from a 2.5 ton industrial truck with the forks raised 12-in. high. They lost control of the extension when it started to slide off the forks. It crushed Employee #1's left fingers in between the concrete floor and the extension. His ring and middle finger were amputated to the first digit and he was hospitalized.
