Investigation Summary
On November 13, 2007, Employee #1, a Dam Tender, was performing maintenance on the inside of a home that was to be later transformed into an office. Employee #1 was standing on a two-step ladder removing paint when she lost her balance and fell to the floor with the stepladder. Employee #1 suffered a compound fracture to her right ankle requiring fifteen screws and four plates. Employee #1 was hospitalized.
