Employee Is Injured When Struck by a Forklift

At approximately 12:10 p.m. on December 4, 2005, Employee #1 had parked a rental van at the base of the warehouse loading ramp of the employer's facility. Employee #1 was attempting to open the rear doors of the vehicle to place bags of ice which a coworker was bringing her on a Raymond (Model Number EASI DR 30 TT, Serial Number.EZD0328603) industrial lift truck. The coworker had parked the lift truck on the lower portion of the ramp approximately 1.5 meters from the van, to help Employee #1 open the van doors. Employee #1 noticed that the lift truck was starting to roll and ran back to it in an unsuccessful attempt to stop its movement. One of the lift trucks forks struck Employee #1 on her right lower leg, pinning it against the bumper of the van. Employee #1 suffered a crushing injury, with a fracture to her right fibula and was hospitalized.
