Investigation Summary
On July 5, 2005, Employee #1 was working with a three-man crew, pumping excess concrete mix back to a concrete mixer through a hopper with 2.5-in. hose. He went up the mixer's ladder and was standing on the platform at the top of the ladder holding the discharge end of the hose over the hopper. When the pump was started, the hose swung and knocked him off balance causing him to fall about 8.5 ft onto an asphalt pavement. Employee #1 sustained a concussion and was hospitalized.
